Counseling

What should I ask my insurance company?

Use the toll-free number or the website on your insurance card.

  1. What are my benefits for “outpatient behavioral health” services (psychotherapy)?
  2. Do I have a deductible?
  3. Have I met the current year deductible?
  4. What is my co-pay/co-insurance per visit?
  5. Am I limited to a certain number of sessions per year?
  6. Is Susan M. Dilsworth, PhD (a clinical licensed psychologist), an in-network provider for my plan?
  7. If Dr. Dilsworth is in-network, what is my deductible?
  8. If Dr. Dilsworth is out-of-network, what is my deductible co-pay?
  9. Is there anything I, or the provider need to do prior to, or immediately after, my first visit (i.e., pre-authorization, referral from physician)? If pre-authorization, be sure to get authorization number (date of authorization) and session limits if applicable.
Can I use my Health Savings Account (HSA)?

Yes.

How will my insurance claims be processed?

We will work with your insurance company to process your claims. You are responsible for your deductible and your co-payment at time of service. We will then submit the claims on your behalf. It’s very important that you contact your insurance company for pre-authorization prior to your first appointment. Be sure to ask if you have a deductible or if you have a co-pay. In addition, verify if you have an unlimited or limited amount of sessions.

I am a new client, what do I need to do before my first appointment?

Call your insurance company to verify eligibility and determine co-pay, deductible and session limits.  Please fill out the six forms located in the upper left hand corner of this page. Note intake form for 18 and over, or 18 and under. Please bring forms with you to the first session. Do not email.
