Yes.
We will work with your insurance company to process your claims. You are responsible for your deductible and your co-payment at time of service. We will then submit the claims on your behalf. It’s very important that you contact your insurance company for pre-authorization prior to your first appointment. Be sure to ask if you have a deductible or if you have a co-pay. In addition, verify if you have an unlimited or limited amount of sessions.
Call your insurance company to verify eligibility and determine co-pay, deductible and session limits. Please fill out the six forms located in the upper left hand corner of this page. Note intake form for 18 and over, or 18 and under. Please bring forms with you to the first session. Do not email.
